Below is the form to sign up for Or Torah Membership and High Holiday Seating. Please read each question carefullyPlease be aware  that space in certain Minyanim sell out quickly and therefore we cannot guarantee your preference will be met. Seats will be confirmed on a first come first serve basis to members whose forms have been accepted as paid or when the member has entered into an agreed upon payment plan with a member of the Finance Committee: Buzzy Schwartz - hmstudio1@aol.com, Cell: (847) 219-5949 and Howard Levy - howard.levy@sbcglobal.net, Cell: (847) 337-7238.

Please note that you do not have to speak to a member of the Finance Committee if you wish to pay half of your membership dues now and half on December 1, 2014. You may submit your payment by check or credit card. Instructions for submitting payment are explained on the Payment Confirmation Page, which can be accessed by filling out the form below and hitting "Submit and Continue".

Membership Information

High Holiday Seats           

Please note that every membership includes a seat for each parent/spouse for Rosh Hashana and Yom Kippur. Each Child's seat cost $75 in total for Rosh Hashana and Yom Kippur. This amount is $75, even if your child will only sit with you at a minyan on one of the two holidays (children of members includes those who live at home and are fully dependent on the member parents).  Guest Seats cost $200 each for Rosh Hashana and an additional $100 each for Yom Kippur.

Building Fund

The building fund obligation is a one-time obligation, of $3,500, payable over 5 years at a suggested $700/year beginning with the member's 4th year as a member. Participation in and payment of the building fund is required in order to be considered a member in good standing.

PAYMENTS ON THE NEXT PAGE AFTER YOU HIT "SUBMIT" BELOW  

Please note that you do not have to obtain approval from a member of the finance committee to pay half of your membership dues now and half in December. 

Payment Options:
1. Credit Card: To pay in full today please select "Paypal" next to payment method and click "Submit and  Continue" below. You will be redirected to Paypal. You do not have to create a Paypal account to  pay with Paypal. To pay as a guest, scroll down to the bottom of the page and follow the  instructions listed. To pay half now and half in December by Credit Card, please follow these instructions:
a. Click on the box next to "Split this Payment
b. Next to the "Split this Payment", select "Two Payments"  
c. Next to the word "Paid" click on the dropdown menu and select Every 5 Months
NOTE: Although the system will allow you to choose to pay every few weeks or months, Or Torah will only process your payment when you select "Every 5 Months". We will contact you if a mistake has been made so that we may rectify it.   Please also note that in order to split your payment, you must create a  Paypal accountThis is similar to registering for an email address on Gmail or Yahoo. The credit card you use to make your first payment will be re-charged exactly 5 months from the day you make your first payment.
2. Pay by Check: Please click on the drop-down menu next to the words "Payment Method" and select "Bill to My Account". Next, hit the submit button below. Please note that your seats will not be reserved until the Or Torah Office receives 2 checks for your membership dues: one check should be current and the other must be postdated to December 1, 2014. Those wishing to reserve seats in Or Torah West  that seats fill up very quickly.
